Welcome to Arrowhead HealthWorks. Our goal is to empower you toward the best possible health. We have found that a clear statement of the financial & appointment policies is helpful to avoid confusion and embarrassment. Sharing this information with you ahead of time will help you understand policies and give an opportunity to ask questions you may have and avoid misunderstandings. There are no "cook-book" approaches to healing--in our belief, among other factors, that is what got the Medical "disease" care System into the mess it is in now. No two people are the same, therefore standardized cookbook approaches are doomed to failure. An integrative wholistic dynamic patient-based method is better.

OUR QUANTUM ENERGY TESTING & ASSESSMENT METHODS:
We may include traditional methods like bloodwork, urine, saliva testing, Thermography or Quantum Energy Evaluation w/TBM® which is a type of Clinical Kinesology, which is an Quantum Energy methood of determining energy imbalances way before any disease manifests. It is faster and simpler than traditional assessment methods and gives instant answers. This is based on Einsteinian Quantum Energy Principles, not Newtonian Principles which focus on chemicals and matter. We now know that absolutely everything breaks down to energy signatures.
Our Quantum Physical Exams are done fully clothed by Dr McCormick who is a home-care Quantum Energy Specialist & Chiropractor who is highly skilled gifted intuitive.
 
By way of information Clinical Applied Kinesology and TBM® which is physiological method of measuring energetic biofeedback signals coming from the perineural nervous system which runs throughout the entire body including the brain. TIME & ENERGY INTENSIVE
*** The Body reveals information in layers, so we expend a tremendous amount of energy, time and effort using Quantum Medicine Techniques to identify and eliminate blockages to healing which have "flagged". Neither the patient nor the therapist has this information ahead of time and it cannot be consciencously controlled. WE'RE LOOKING FOR UNDERLYING CAUSES/BLOCKS NOT DISEASE TAGS. It is gleaned through the muscle response information revealed through the perineural nervous system (also called the Living Matrix) which is the master control system in the body. We employ a modified form of Clincal Applied Kinesology & TBM® (Total Body Modification) which is a form of BioFeedback, however we don't use equipment generally which is only as good as it's programming. Instead we are blessed to have the medical intuitive services and extensive training and exquisite sensitivity of Dr.McCormick to help us with identifying imbalances, infections, toxins, etc.
 
Through this process one may find physiological, structural, psychological, emotional, spirtual, microbial, environmental poisions and other blockages. As blocks are found, they are corrected energetically so that we can open all the channels so the body can heal itself... We're not focused on "disease tags" which actually can do more harm than good.
 
QUAMTUM ENERGY EVALUATION & TREATMENT IS TIME INTENSIVE... Our evaluations are done with both Dr.Bormann & McCormick since it takes us both to get answers and do treatment, sometimes simultaneously. In order to do this correctly, it means we give a 100% of focused time and attention so one must allow much more time than what people are typically used to. It should be done right or not at all. It is more expensive to practice healthcare this way and Dr.McCormick can't see as many individuals but to him ethically, there is no other way to wokk. This is not a 5 minute standardized therapy "cookbook" office visit. None of us knows what we're going to find exactly. Having an open mind and going with the flow is recommended. We may need to actually go into 2 days depending on how long standing things are and what they're body is revealing, so we will typically spend 4 hours the first day (we don't want everyone getting exhausted and we need to respect other peoples time and committments) and then if more work is needed, we bring them back the next day to finish up. The second day is billed as a follow up visit which is less expensive.

SCHEDULING POLICIES
Due to limited space, we're by Appointment Only. General Office Hours are M-F 9-6, Sat. 10-4; Closed Sunday, Holidays, Snow Days. Call to verify if we're on time or behind, etc.
Appointments are first come first served. We will do our best to accomodate schedules and life events but sometimes it just isn't possible due to consideration of others time and responsibilities Dr.Marvin's availability will fluctuate since we're not the only place he consults for...he is in great demand as a home-care doc and travels all over the US treating "incurables". His schedule is subject to change based on case parameters. We recommend if he is available, to take advantage of it.
 
1. Go to the following links, fill them out and print them and fax the completed and signed paperwork to our office. Appointment Request Form , History Form and Consent Form
2. For appointments less than 2 weeks out, 100% prepayment is required in advance. Designate method on Appointment Request Form.
3. Appointments for dates more than 2 weeks out are required to pay a non-refundable appt. reservation deposit of $100.00 and the balance is due at the appointment. This must be enclosed with all the appropriate forms.
 
NEW PATIENT CANCELLATIONS & RESCHEDULING POLICIES:

Cancellation Policy:
New Patient Appoints who have prepaid in advance and who cancel within 24 hrs. of their scheduled appointment time will be refunded any fees less the $100.00 Non-Refundable Appt.Reservation Fee.
 
Rescheduling:
New Patients: Any appointment, which we're notified not less than 1 week advance notice that needs to be rescheduled within 14 days of the original appointment. Prepaid fees are applied to the new appointment time one time if rescheduled within 14 days. Otherwise it is considered a cancellation and those policies apply.
Existing Patients: An appointment which we're notified less than 48 hrs. ahead of time which needs to be rescheduled to another time within 30 days one time, there is no fee. Otherwise, for missed appointments and "no shows" we assess a $20.00 fee.
 
NOTE : Paperwork faxed to us in advance of the appointment which is incomplete must be completed prior to completing the appointment. Our office will contact you if there are any discrepencies in the fiancial or history paperwork, such as no credit card number with authorization or lack of signed consent forms, missing labs, missing pages of history, etc.

** NO CHECKS ARE ACCEPTED AT THE TIME OF THE APPOINTMENT. We only accept personal or business checks for advance prepayment.
 
CREDIT CARD POLICY: We accept Visa, Mastercard, Discover. We also accept Visa & Mastercard Debit Cards. There is a 3% facilitation fee charged to accept Credit Cards however.
 
NOTE: Foreign Credit Card Holders: some credit card merchant processors will not take cards from certain countries and/ or will hold up the funds until clearance by the foreign bank. There may be a delay for receipt of funds sometimes by 2 weeks depending on the country and the merchant provider's policies, and the amount of the charge. Cards need to be cleared with our processing house first before we can accept them if they're from South America, parts of Europe, Asia, the Middle East.
 
CHECK POLICY: Our bank requires a 7-10 business day lead time for clearance of out of state funds from personal or business checks. Prepayment of the appointment with a personal or business check is o.k. to send but the appointment will not be secured until verification of funds. All NSF checks will be charged a $35 NSF fee by us in addition to fees by the issuing bank, and the appointment canceled until rectified and then rescheduled for a later date.
 
NEW PATIENT PREPAID FEE REFUND POLICY: All new patients who are not Quantum Comprehensive New Patients who have to cancel within 48 hrs. of their appointment will be returned their fee less the $50.00 Cancellation Fee. There is a $100 non-refundable retainer to hold the appt. block dates for New Quantum Comprehensive patients.
 
INSURANCE:
Unfortunately due to the nature of our work, we cannot accept insurance for direct payment. There are no codes for what we do. We can give a detailed receipt for the patient to attempt direct reimbursement from their insurer. For Europa patients, we can hook the patient up with our contract Insurance Administration Billing Firm in Texas which specializes in foreign clinic recoveries and gets good results. They typically charge a 20% fee which is only collected if they get a recovery.
 
All charges will be the responsibility of the patient or the responsible party as acknowledged below to be paid according to contract terms or at the time of services rendered.
All financial transactions here are for Arrowhead HealthWorks services only unless otherwise arranged on a case by case basis.

 MISCELLANEOUS
APPOINTMENT PAPERWORK REQUIREMENTS
In order to do the best possible job at the consultation, we must have copies of all recent pertinent labs, x-ray reports, ultrasound reports, CAT scan reports, MRI reports, etc. and Pathology reports as well as pertinent chemistry panels, etc. We may need to order additional tests to have a complete picture before determining, together with the medical team, the appropriate options. If no diagnostics are available, we will have to order some before finishing the consultation, and a follow up consult will be scheduled to go over the results and discuss the suggested treatment options. You'd be surprised at the sloppy or inadequate diagnostics that we see come through our doors...
 
FAMILY, ETC.
Due to limited physical space, we can't accomodate small children or extra family members since we don't have a waiting area in our current space except to sit outside in our hallway. There are nearby restuarants, Starbucks, Gourmet Market, library, parks, lakes, movie theature, shopping, etc. Also, No Smoking, or Alcoholic beverages is permitted in the building.
